access to mysql (no replies)

I am trying to upload Microsoft Access 10 “quotes” table in a “quotes” database on Windows 7 32-bit to a MySQL database newworld_creatingnewworlds.org as table “quotes”.

I am using the ODBC Connector 5.2 unicode Connector. The host MySQL is Version 5.0.96-community-log. PHP is version 5.3.275.27 running on Linux with Apache. My domain IP is 69.167.158.105 (this is for domain creatingnewworlds.org.
On execution, I get the error message shown in the attached that the connection has failed.
